چکیده
Abstract Funding Acknowledgements Type of funding sources: None. Background Catheter ablation is recommended as first-line therapy for patients with symptomatic typical atrial flutter (AFl). Although the conventional multi-catheter approach standard care cavotricuspid isthmus (CTI) ablation, a single-catheter was recently described feasible alternative. Purpose The present study sought to compare safety, efficacy, and efficiency single vs AFl ablation. Methods In this randomized multicentre study, consecutive referred (n= 253) were enrolled multiple CTI arm block confirmed analyzing PR interval (PRI) on surface ECG previously by Madaffari colleagues: briefly, PRI defined time between pacing spike onset QRS complex, measured during (10 V, 1.5 ms) at stable cycle length from tip catheter placed 5 o’clock (medial line), 7 (lateral 9 position. assumed when: (i) >80ms longer than that sites 5, (ii) shorter (Figure 1). Procedural follow-up data collected compared two arms. Results 128 125 assigned arms, respectively. arm, procedure singificantly (37±25 48±27 minutes, p=0.002), required less fluoroscopy (430±461 712±628 seconds, p<0.001) radiofrequency (428±316 643±519 p<0.001), achieving higher first-pass rate [55 (45%) 37 (31%), p=0.044], arm. After median 12 months, 11 (4%) experienced recurrences [5 in 6 (5%) P=0.99]. No differences found arrhythmia-free survival arms (log-rank= 0.71). are summarized Figure 2. Conclusions Recurrence-free not inferior multiple-catheter approach, reducing procedure, time.

متن کاملAn Approach to Catheter Ablation of Cavotricuspid Isthmus Dependent Atrial Flutter
Much of our understanding of the mechanisms of macro re-entrant atrial tachycardia comes from study of cavotricuspid isthmus (CTI) dependent atrial flutter. In the majority of cases, the diagnosis can be made from simple analysis of the surface ECG. Endocardial mapping during tachycardia allows confirmation of the macro re-entrant circuit within the right atrium while, at the same time, permitt...
متن کاملEffect of isthmus anatomy and ablation catheter on radiofrequency catheter ablation of the cavotricuspid isthmus.
BACKGROUND Cavotricuspid isthmus (CTI) characteristics are rarely documented when comparing catheters in radiofrequency ablation (RFA) of atrial flutter (AFL). Our objectives were (1) to evaluate the impact of CTI morphology and length on ablation procedures and (2) to compare the efficacy of an 8-mm-tip catheter with an irrigated cooled-tip RFA in the subgroup presumed to be more difficult to ...

Randomized comparison of the continuous vs point-by-point radiofrequency ablation of the cavotricuspid isthmus for atrial flutter.
BACKGROUND Achievement of complete conduction block in the cavotricuspid isthmus (CTI) is a curative ablation technique in patients with common atrial flutter (AFL). The present study was a prospective comparison of the efficacy of 2 ablation strategies in patients with common AFL: the continuous and point-by-point radiofrequency (RF) delivery techniques. METHODS AND RESULTS Forty patients wi...
